The purpose of the Salt Lake County Human Services Department is to make a positive difference in the lives of Salt Lake County residents.

As the population within our County continues to grow so does the demand for services. However, the resources to provide quality services to all our citizens are limited. As we look toward making decisions about human service programs within Salt Lake County we’d like to invite you to share your opinion of what should be the focus.

Please take a few minutes to answer some questions to help us identify your service priorities within Salt Lake County. Each of the areas below represents a separate questionnaire. Please feel free to answer any or all of them. All information you share will remain anonymous and confidential.
